MLMC Releases New Advance Care Planning Guide

We are very excited to announce the unveiling of our brand new Advance Care Planning Guide, a tool to help you think about, talk about, and write down what matters most to you if you become seriously ill or injured.

This new resource combines many of the advance care planning (ACP) tools available on the My Life My Choices website, as well as a few new things, into a single multi-page document that you can download and customize.

Once downloaded, local health care providers can quickly and easily, with the click of a mouse, add their contact information to the back cover as a resource for more information on advance care planning in their area.

The booklet also contains a fillable copy of our popular ACP Bubble Map, which allows anyone to prepare for the ACP process by filling in some answers to the question “what matters most to you?” Once downloaded, the bubble map can be filled in just like the back cover.

This booklet was created with help from the South Carolina Hospital Association, South Carolina Medical Association, the Physicians Foundation, and The Carolinas Center, and will help guide you in talking with loved ones about what matters most to you should they have to make health care decisions for you in the future.

It will also help you choose a health care agent and document your wishes for health care if you are unable to speak for yourself. This information will help your health care agent to honor your decisions.

ACP is not just about planning for the elderly, but it is for anyone 18 years or older. It is making decisions about the care you would want to receive if you become unable to speak for yourself due to a sudden, serious accident or illness.

Please take time to read this booklet carefully and use the information to discuss what matters most to you with your loved ones, health care agent, and your doctor.
