My Life My Choices has teamed up with Vynca Health to provide an eRegistry to store Physician Orders for Scope of Treatment (POST) forms.
The My Life My Choices eRegistry provides a platform to ensure health care preferences and end-of-life wishes are electronically available to all clinicians across the state. This eRegistry provides organizations a solution to not only meet the advance care planning requirements under the new Physician Orders for Scope of Treatment (POST) legislation, but also an organization’s broader advance care planning needs.
Our registry gives clinicians immediate access to advance care planning documents within the registry, ensuring they can provide treatment that aligns with patient end-of-life wishes.
The goal of this technology is to help health care professionals honor an individual’s goals of care across all points of care.
How It Works
Registered users can access the registry via a web portal, gaining immediate access to advance care planning documents, regardless of where they were completed.
eDocument Completion
Electronically complete and submit ePOST forms. By utilizing ePOST, clinicians avoid discordant choices and ensure patient wishes are clear and consistent.
Patient Matching
The eRegistry is powered by Vynca and includes its award-winning patient matching solution. Any new documents, no matter where they are created, will be matched to the appropriate person and available to all clinicians throughout the state.
Engage Patients
Engage in structured conversations with patients with a serious illness, supported by an evidence-based conversation guide to help clearly identify goals of care.
Access
Advance care planning documents are digitally accessible through the web portal regardless of where the documents are completed.

Gaining Access to the eRegistry
There are three steps to gaining access to the eRegistry:
Complete the registration form for new organizations
Attend an orientation session
Register users within your organization
Registered organizations will have access to technical assistant and support via info@mylifemychoices.org.
The eRegistry allows your staff to not only securely store and retrieve advance care planning documents, including advance directives. It can serve as a single source of truth for care preferences by allowing clinicians to engage their seriously ill patients in shared-decision making, and access the patient’s advance care planning documents, including POST, at the point of care no matter where the form was developed.
