Having Conversations That Matter: A Community Advance Care Planning Symposium.

Join us Saturday, November 23, at Prisma Health Baptist in Columbia for a free 3-hour symposium on Advance Care Planning—including continental breakfast and a panel discussion.

This event, sponsored by Prisma Health Home Care & Hospice, will focus on helping patients and families plan ahead for their health care.

If you are interested in learning more about:

  • Future health care choices
  • How to discuss your decisions with loved ones, religious/spiritual leaders and physicians
  • Completing an Advance Directive for yourself (or loved one) to ensure medical team honors your choices
